Our Team:
Halton Borough Council is looking to employ a Social Worker to work within Halton’s Hospital Discharge Team, based at Whiston Hospital.
The Hospital Discharge Team is and an Adult Social Work Discharge Team responsible for completing timely strengths-based assessments, to facilitate hospital discharge for adults over the age of 18 years. The focus of the role will be working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence).
The Social Worker role involves completing assessments and personalised, strengths-based support plans to facilitate safe and timely discharges for people with lived experiences. You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties), whilst challenging discrimination and promoting independence.
Hours: 37 per week
Your Contributions:
You will have knowledge of relevant social work legislation such as The Care Act, The Mental Capacity Act and a good understanding of their principles. Ideally you will have knowledge and some experience in risk assessments and risk management.
You will be an enthusiastic and committed team player and be dedicated to multi-agency team working within the hospital setting. Furthermore, you will be able to manage and organise your own case load.
Applicants must have good interpersonal and communication skills, a flexible approach and a willingness to learn new skills and continually develop. Ideally you will have good IT skills.
